SocialRx Partners with LifeBridge Community Services to Bring Social Prescribing to Bridgeport and Fairfield
Partnership connects LifeBridge clients with local arts and cultural engagements that support connection and well-being as part of their care
BRIDGEPORT, Conn. — September 29, 2026 — SocialRx, a social prescribing platform connecting members to community-based arts, culture, nature, and movement activities, today announced a new partnership with LifeBridge Community Services, a Bridgeport-based nonprofit providing trauma-informed behavioral health services to youth, adults, and families across Fairfield County, Connecticut. Social prescribing is an evidence-based approach to care that recognizes the important role relationships, meaningful activities, and community connection play in mental health and overall well-being.
Through the partnership, LifeBridge's care teams can refer clients to personalized, community-based activities that support well-being alongside clinical treatment. Clients receive ongoing, individualized support finding programs and experiences that match their interests and needs, with the goal of addressing social isolation as part of a broader approach to care.
